Clinical Care at Home

The Clinical Care at Home Team are a multi-disciplinary team of therapists, nurses and support workers, who will attend a patient in their own home to organise and, if appropriate, deliver ‘hospital at home’ therapy and care services.

The Service has two parts:

Rapid Intervention

Patients accepted for care under Rapid Intervention will include those with a non-life threatening condition, who would alternatively be admitted to hospital

Teams assess and organise nursing and social care for patients with acute conditions and, if necessary, will organise diagnostic tests, arrange admission to a community bed or if appropriate a hospital .

The period of care delivery from the Rapid Intervention component within the community is up to a maximum of 72 hours.

Rehabilitation and Therapy

The rehabilitation and therapy part incorporates 4 aspects:-

Intermediate Care – operates 7 days a week and aims to support patients following a physical injury or an acute medical condition through activities of daily living. Physiotherapists and Occupational Therapists work with re-enablement support staff to enable patients to regain abilities and independence within their own homes.

Community Physiotherapy – provides assessment and treatment for patients who are having difficulties with their mobility.

Community Occupational Therapy – provides assessment of a patient’s main place of residence and makes recommendations for equipment or ways to promote independence in everyday activities.

Neuro Rehabilitation Team – provides a specialist rehabilitation service to patients who have a newly diagnosed neurological condition or exacerbation of an existing neurological condition and who would benefit from rehabilitation to improve their quality of life.
