Older Peoples Community Mental Health Teams

Older Peoples Community Mental Health Teams (CMHT’s) support older people requiring specialist mental health services.

CMHT’s are comprised of community psychiatric nurses, occupational therapists, psychologists, approved social workers, support workers, psychiatrists and staff grade doctors.

The teams work with patients in their own homes which can include; warden controlled complexes, private and local authority residential homes, private and NHS nursing homes, and the Trust’s community resource centres.

CMHT’s perform a variety of working including:

  • Assessment
  • Care planning, co-ordination and monitoring
  • Rehabilitation
  • Occupational therapy
  • Domiciliary support

All initial assessments are carried out by a qualified worker and if ongoing care is required a care co-ordinator is assigned.

The Community Mental Health Teams have an open referral system and referrals are also accepted for people under the age of 65 with early onset dementia.
