Rapid Response is a dedicated admission avoidance and early hospital discharge team providing intensive nursing care and monitoring for patients in their own home during a short term illness or exacerbation of a chronic condition that would otherwise likely require hospital admission.
The team is also responsible for early discharge schemes in both Cellulitis and COPD management from Southend University Hospital, to provide and monitor treatment within the patient’s home following hospital discharge.
The team also work closely with the Oncology unit at Southend Hospital, providing home visits to undertake phlebotomy, nursing observations and assessment and reassurance; preventing patients from attending the A&E department. Guideline length of stay with the team is five days but this is flexible according to the individual needs of the patient.

365 days per year between the hours of 08.00- 22.00, with new admissions being accepted 08.00 and 20.00, seven days per week.

How to access the service
Referrals are accepted from local hospitals, including A&E, DAU and AMU, Ambulance service, GPs, DNs, Community Matrons, Social services and other healthcare professionals. Patients can self refer if known to the service.
