Children’s Diabetes Service
The children’s Community Diabetes Service is a nurse led service that facilitates self/family-management, enabling children, young people and families living with diabetes to make the necessary adjustments to remain well, reducing mortality, morbidity and the need for hospitalisation. The Service provides community based care and support for children, young people and families with a diagnosis of diabetes and works closely with secondary and primary care services to ensure that the Diabetes care pathway is implemented appropriately according to patient need.
Where the service is based
The service is based at Rochford Hospital, Rochford.
The Service is delivered through a combination of;
- Nurse Led satellite clinics in the community
- Structured education programmes for patients and their families, care staff, health care professionals, Post grad and nursing students
- Domiciliary visits
- School visits
- Multidisciplinary clinics at Southend Hospital
Hours of Operation
The service operates from Monday – Friday 09:00 – 17:00 hrs
The service also offers an out of hours service to children and young people already on the case load

How to access the service
Referrals from other health professionals are made to the team at the point of diagnosis and first contact will be made within 24 hours of receipt of the referral.
