Service Information
West Essex Hospital at Home offers the same care you would receive in hospital, but at home or your usual place of residence.
Care is safe and will be managed by our dedicated consultant-led clinical team, who also work closely with specialist services.
We will assess your individual needs and organise any tests, treatments and further care you need while you are with us.
We will also arrange for any adult social care and equipment you need to help you live independently at home.
Our clinical team will contact you and monitor your progress every day and as often as needed. They will do this through a combination of phone calls, home visits and taking readings from easy-to-use equipment.
You may be asked to use monitoring devices to check your blood pressure, oxygen levels, temperature, or weight. These will be provided, along with help and support.
West Essex Hospital At Home cares for patients for up to 14 days. We will organise referrals for any ongoing care you need after you are discharged from our service.

We accept patients who are aged 18+, registered with a west Essex GP, are assessed by a clinician as suitable for care under the West Essex Hospital at Home, and consent to receiving care under the service.
