The Community Nursing Service is an essential part of the local Southeast Essex (SEE) health and social care system and can make the difference between people being able to stay at home rather than being admitted to hospital or nursing home care. The Community Nursing Service team provide a domiciliary flexible service to housebound patients to assess health care needs and deliver a wide range of nursing interventions.

Opportunity to join EPUT Community Respiratory Nursing Team supporting patients with disease management, admission avoidance, early supported discharge, early diagnosis of COPD supporting breathlessness hubs.
EPUT works closely with both primary care and acute respiratory medicine both within the South East Essex and across the wider MSE sector.

The successful candidate will be an RSCN or RN Child with acute paediatric experience; they will have an in-depth knowledge of providing family centered holistic nursing care .The individual will be organised and be an excellent communicator able to cross boundaries with the MDT and families alike.
