West Essex Community Specialist Diabetes Service

Summary

A specialist level 4 consultative service for adults with diabetes led by diabetes practitioners who are diabetes specialist nurses or diabetes specialist dietitians.

Service Information

The team provide the following support: 

  • DAFNE (Dose Adjustment for normal eating) – structured diabetes education for adults with Type 1 diabetes. Click here for more information about upcoming course dates.

  • X-PERT – structured diabetes education for adults with Type 2 diabetes and delivered by the West Essex Community Dietetic Team.
  • One to one support for adults with Type 1 diabetes and, where appropriate, complex Type 2 diabetes.
  • Support and advice for health care practitioners.
  • Diabetes support for west and central Essex mental health inpatient units.
  • Diabetes support and advice for community district nursing teams, and nursing and residential care homes.

Referral Information

Referrals via West Essex Care Coordination Centre are assessed under service criteria for acceptance. Where referrals are not appropriate, an advisory response via letter or direct message will be provided for the health care professional.

Adults with Type 1 diabetes can self-refer for non-urgent support via telephone or email.

GPs and non-medical prescribers will need to complete signed consent forms when referring adults being supported by the community district nursing team.
