Avocet Ward
Summary
Avocet ward is a community inpatient rehabilitation ward, providing a service to patients over the age of 65 years.
The ward has 21 beds, 2 of which are only open for winter pressures and increase in service demand. This includes 5 isolated side rooms each with en-suite and 4 Bays consisting of four beds and bathroom in each bay.
Service Information
We provide an integrated service, with an in-depth, holistic assessment of the psychological, social and physical needs of each referred individual.
Based on our expert assessments and in partnership with the patient and their family / carers, we provide each patient with an individual package of treatment and support, tailored to their assessed needs and reviewed regularly.
Our team
We place a strong emphasis on the multi-disciplinary team approach and patient care plans and reviews are agreed daily with the input of all team members.
We have a ward-based Consultant Geriatrician, supported by a ward doctor. Each patient will have a fully-qualified named Nurse and Healthcare Assistant per shift.
Our team also includes ward-based Psychotherapists and Occupational Therapists). Input is also available from other specialists as required, such as, Speech and Language Therapists, Dietitians and Tissue Viability Nurses.
