Early Supported Discharge Service (ESD)
Summary
The multidisciplinary team provides early, intensive, specialist assessment and rehabilitation in the community for Stroke Survivors (18 years +) for up to six weeks.
Service Information
The team includes physiotherapists, occupational therapists, speech and language therapists, a psychologist and therapy assistants.
Patients are offered intensive rehabilitation which aims to facilitate return to previous daily life and independence focussing on patient-led goals. The ESD team has strong links with the Stroke Association and can signpost to other local Stroke voluntary or charitable services.
Patients with a diagnosis of stroke are followed up six months from the date of their stroke to review and discuss their ongoing recovery, and directed or referred to long term sources of support if required.

Rehabilitation is provided in patient’s homes/communities or as outpatient sessions at community hospitals. Sessions may also be offered virtually in a single or group setting.
