Integrated Mental Health Service - West Essex

Summary

Mental health professionals at the GP surgery will assess your mental health and offer short term support. They also refer you to the right services for further help.

Service Information

We are here to help people who have mental health needs and create a suitable support plan at an early stage.

Working together with your GP and our colleagues, we can give you advice and support to help you to improve your mental health holistically.

What should I expect at my first assessment appointment?

We will ask you what you are currently finding difficult at the moment and ask you to rate how satisfied you are with the different parts of your life.

We will ask you about your risk. Some questions might be hard to answer, but please take your time and answer truthfully. Let us know, if you find this difficult.

From the results of the assessment, we will create a plan to help you. We will share this plan with you, your GP and/or referrer.

Your plan

We have access to resources and organisations that could help you.

Support may include advice, referral to a service that suits your needs, support from the Mental Health Coach, Health and Well-Being Coach or Mental Health Pharmacist for medication advice.

We can also offer short-term interventions. We will go through the plan in detail with you, so you will have the chance to ask any questions at this stage. If it is needed, a month after your assessment we will call you to see how the plan is going and if you have any questions or concerns. Once this care has been completed we will transfer you to the most suitable service for your needs at this time. However, if things have changed with your mental health and you need further support, you can be reviewed by us at any time.

Confidentiality

We treat confidentiality very seriously. With your consent, we will only share information about you with your GP and other NHS Mental Health Professional/s who is involved in your care.

If you are at risk of harm to yourself or others, we may have to talk to other mental health experts or the right authorities to make sure you get the right support.

We are entirely transparent with this and are happy to discuss confidentially in further details if you have any concerns.

Referral Information

If you require support, please speak with your GP Surgery who can refer to our service.

If you or anyone you know are in crisis, please call 111 and select the option for mental health crisis.
