Intensive Outreach Team

The Intensive Outreach Team is a combination of the Assertive Outreach and Rehabilitation teams. 

The IOT is a multi-Disciplinary team with specialist Nursing, Occupational Therapy (OT), Psychology and Support Workers providing care to people who have:

• Evidence of severe and enduring mental illness including noncompliance of medication and disengagement
• Clients who require more intensity than generic community secondary services can offer
• High frequency and intensity of challenging behaviour
• Frequent admission to Inpatient units in the last two years
• People with a diagnosis of Severe Mental Illness who also have a diagnosis of a Personality Disorder (PD)
