Summary
Exercise and education based course completed twice weekly for six weeks.
Service Information
Rehabilitation programme for patients that are formally diagnosed with a respiratory condition e.g. Asthma, Chronic Obstructive Pulmonary Disease.
This programme provides a structured exercise programme that you complete at your own pace with an education programme that improves understanding around self-management of your breathing issues.
This can now be provided in a variety of settings including your own home and online.

Referral Information
Referral from GP, healthcare professionals and local hospital(s).
- Main diagnosis of COPD but other chronic respiratory disease will be considered.
- To have had spirometry carried out within the last 12 months or noted decline in respiratory health.
- FEV1 below 50% predicted value.
- Declining functional capacity (MRC grade 3 or above).
- To be registered with a G.P. practice within the commissioning area.
- To be able to mobilise a minimum of 20 metres unassisted.
- Be clinically stable
- On optimised medication
- Be motivated to complete twice weekly for six weeks.
Referrals for the pulmonary rehab programme are made using the pulmonary rehab referral form and then emailed to the team using the address, epunft.pulmonary.rehabilitationreferral@nhs.net
